Just to clairfy. Our QHIO sent us a

questionnaire. We practice most of the

questions listed but lacking the written

policies. This is the hiccup as we cannot

proceed with the next step because our

QHIO are requiring IT written policies. I

reqeusted if they have a template so we

can have those in place but they are not

able to help. Do you have suggestions

who to approach so we can meet the IT

written policies?

Hi Jackie. I'd love to see the questionnaire if

you don't mind sharing with me. Otherwise, I

can ask your selected QHIO directly if you let

me know who the QHIO is (I often talk or email

with the QHIOs daily).

TA GPAs should be signed by an individual who

can accept responsibility on behalf of the

Milestone 1 must be accomplished within 12

months of your grant award date.

Hi, Khoua -- do all the signatories have to

sign AT THE SAME TIME for an umbrella

organization?

Umbrella applications required having each

DSA Signatory sign as a part of the application

submission. In order to meet milestones, all

DSA Signatories need to have met the

milestone, we do not consider an Umbrella

milestone complete until all DSA Signatories

have met the milestone.

Hi Tracy, this means that a contract or

agreement for an HIT vendor, external

personnel, and/or other external/internal

resources has been procured.

What if we do not use all of the funds

available for the grant when we reach final

milestones? Do we have to return all

funds, or just the unused portion?

If you have met your outcomes, CDII will not ask

for funds to be returned.

If you were awarded $50K, for TA Milestone 1

you must attest to contracting with a HIT

vendor, external personnel, and/or other

external/internal Resources have been procured

accounting for at least 50% of the grant budget,

so at least $25K.

Do all 4 grant outcomes need to be

completed for Milestone 2 or just one of

them?

Hi Maia! All outcomes that the grantee chose on

their grant application must be met in order to

reach milestone 2. For example, if the grantee

choose 3 outcomes, they must achieve all 3

before they are able to attest to milestone 2.

Hello, I received an email from

[email protected] on February 20th which

provided an update on the status of the

program. It was stated a new Participant

Directory that was launched on January

31st. As a signatory, we were encouraged

to update our participant choices on this

portal as soon as possible. The directions

were not very clear and I am unsure what

was menat by "choices" and how this will

effect my orgainzation's current standing as

a signatory and current DxF grant

awardee. Is there any guidance that ca nbe

provided on the urgency of this task and

how I can appropriately complete this task

on the Participant Portal?

The Participant Directory will not impact your

Grant. CDII can look into details for you and

reach out in the upcoming days.

process of submitting progress reports, and plan

to continue to provide necessary support to both

QHIOs and TA grantees.

OTher question is if I have a current EHR

vendor that is not meeting requirements

and I decide to switch to a new EHR can

funds be used for transfer from one EHR to

another (i.e. break contract, transfer of

data, start up costs etc)....pain and

suffering (: just kidding on the final.

Acquiring or updating HIT or EHR technology is

an acceptable use of funds. You can find out

more on the acceptable uses of funds in the

Grant Guidance document.

Hi Swati, all grants have been awarded and

CDII does not plan to hold another round of

DSA Signatory Grants.

Neither CDII nor PCG maintain a vendor or

EHR list. You might want to look at the ONC's

Necessary documentation for progress reports

and grant awards is outlined by Outcome in the

Grant Guidance document on page 45.

How can we Umbrella Grantees have co-

signed documents for Milestone progress

reports? For example, if we have 27

signatories. One document with all 27

signatures, or 27 separate files?

We (CDII and PCG) are able to work with you if

you need out-of-band, however we are fairly

flexible. For example, if you have 27 separate

signature pages that you combine into one pdf

to upload to the GrantsConnect portal or if you

have to upload 27 different pdfs, we're willing to

work with you.

The electronic signature is all that is required.

You will prompted to provide one when

selecting 'Submit'.

something procured? When a vendor is

selected and contract is signed? When a

consultant is hired? When an internal

resource is allocated to the project? thank

Thanks for the clarifying question, Tracy! A

signed contract with a vendor or the allocation

of an internal resource of a project would be

sufficient in reaching Milestone 1, as long as the

Grantee has only chosen to meet Outcome 1

("Identifying and Contracting with a Technology

Solution"). If a grantee identified that they would

meet more than one outcome, they would need

to meet all outcomes before they can attest to

milestone 1.

coordinating the grant for us but even after

registering an account at the DxF Grant

Portal at https://app.blackbaud.com/, I am

unable to see the grant information. How

can I view the grant information?

An applicant on the existing application must

What exactly is a process report?

A progress report is a report all grantees must

submit to provide CDII with an update on

progress and/or attest to the completion of a

milestone. A progress report that includes a

milestone achievement will trigger a payment.

mindful of grant funding maximums per our

costs, grants are intended to supplement costs.

What if a transition were to happen

between Milestone 1 and Milestone2 from

a solo practice and the physician and

patients join a larger group or a hospital

network like Stanford or Sutter or

Thanks for this information. The Guidance

Document mentions execution of a grant

agreement that has the specific milestones

that we submitted in our application. Is that

agreement the GPA that you discussed

that is due next Friday? In the grant

acknowledgement letter that we submitted,

I don't see our specific milestones included.

Hi Catherine! Thanks for your clarifying question

- the grant agreement is the very first thing you

sign, that essentially makes you an official DSA

Signatory Grantee. Round 1 and Round 2

grantees have already signed this, but newly

awarded Round 3 applicants have until next

Friday to sign this GPA. Milestones are different

from TA and QHIO Grantees. TA Grantees

identified their chosen outcomes in their

application, and these will be what they work

towards in their milestones.

A Progress Report will not present in the

GrantsConnect portal until after you have

signed your Grants Program Acknowledgment.

There is about a week's time of administrative

processing internally before your application is

marked as a grant with a signed GPA. After

that you should be able to access Progress

Report in the Grants Connect portal. If you

have met the milestone (as you described) you

can submit for M1 as soon as you see the

Progress Report in your Application ID account

in Grants Connect.

Hi Geselle, thanks for your question. If you have

submitted your GPA (Round 1 & 2 grantees

have already signed, Round 3 grantees have

until 3/8 to sign), then you are officially a

grantee! If you are ready to submit a progress

report indicating a milestone achivement, you

have until May 31st to submit in order to

achieve expedited funding. If you are not yet

ready to do so, you can wait until June 30th,

Hi Josh, thanks for your clarifying question! For

the GPA specifically, all DSA Sigs *do not* need

to sign - just the Umbrella lead. They also do

not need to co-sign progress reports that *do

not* attest to a milestone.

They do have to co-sign when a milestone is

achieved and indicated in progress report.

Onboarding Grants, grant dollars go directly to

the QHIO. It is the responsibility of the DSA

Grantee and their chosen QHIO to determine

how that funding will be used.

Hi Saurin, thanks for your question. Grantees

(or a QHIO Onboarding Grantee's chosen

QHIO) will be required to submit progress

reports on a quarterly basis starting June 30th.

Disbursement of funds will only be provided

after a milestone is achieved. This will be

indicated in your progress report.

Hi Hannah, thanks for your question! Yes, even

if you cannot report a milestone achievment in

your progress report, Grantees must submit the

required progress reports until they achieve

milestone 2. You can find these reporting dates

on page 43 of the guidance document:

Progress Report deadline?

Hi Jason, thanks or your question. The first

*required* progress report deadline is June

30th. Optional progress reports indiciating an

achievement of a milestone can be submitted

through May 31st to access expedited funding.
