Admissions Response Form
This form is only to be submitted by students who have been admitted for the upcoming semester.
Please type your responses in the fillable fields below and return this form to inform us whether or not you will be attending the
College of Staten Island (CSI). If you accept our offer please include your non-refundable $100 commitment deposit to our
Recruitment and Admissions Office.
By submitting your commitment deposit you secure your enrollment to CSI. This deposit will be applied toward your tuition charges
(this is not an additional fee) and will allow you to be scheduled for academic advisement, registration and orientation; therefore, you
are strongly encouraged to send in your deposit as soon as possible.
Commitment deposit deadline for Fall semester: May 1
If you are admitted after May 1, your commitment deposit is due 15 days after the date you receive your admission notification.
Commitment deposit deadline for Spring semester: December 15
If you applied for financial aid using the Free Application for Student Aid (FAFSA) and your Student Aid Index (SAI) is $2950 or less,
you may be waived from the commitment deposit.
If you are accepting our offer of admission, payment for the commitment deposit can be paid in person or mailed to:

The $100 commitment deposit must be satisfied by a check or money
order made payable to the “College of Staten Island”.
